Document Submission Form

If you have not already done so, submit an ID photo here. This photo will be used to create your student id and must be:

- In color
- Composed to include the full head
- Taken within the last 6 months to reflect your current appearance
- Taken in front of a plain background
- Taken in full-face view directly facing the camera

ALL STUDENTS
1) A scan of the page of your passport with your identification information. 2) Your Birth certificate - original copy and official French translation (a translation not required if your birth certificate is in English or Spanish)

NON European Union (EU) STUDENTS
In addition you must upload a scan of:
