Summer Program Student Information Form

This form is mandatory for all students enrolled in PCA summer courses. You will be asked to upload the following in this form:

• A passport size photo of yourself
• A scan of your passport and (if applicable) your visa to enter France
• A signed Student Participation Waiver
• For students staying in Citadines Place d’Italie: a signed Summer Housing Release Form
